Care Team Collaboration

Transitions for any of us can be difficult, regardless of age and stage, whether it be attending a long day care centre, kindergarten or ELC, starting Prep at primary school, moving schools at different year levels or moving from interstate.

For some children transitions can raise separation issues, emotional and/or behavioural challenges. It is imperative that children and their families are supported through the transition process with consistent care, compassionate communication, and importantly, understanding.

When it comes to understanding and navigating starting at an early childhood service or primary school, particularly if we have a child who is neurodiverse (with a diagnosis or challenging behaviours are being explored), the process can often be overwhelming for children and families, no matter when the transition occurs.

The Sunflower Village can help.

From advocating for the right supports for your child, to understanding Individualised Learning/Education Plans, or navigating the unexpected territory of finding specialists for referrals, this can often be an overwhelming time.

In Stacey’s dual roles of Transition Leader and Learning Diversity Leader, Stacey’s warmth and care of families, and ability to coordinate connections between all critical members of a child’s Care Team (including Allied Health professionals) ensured that the transition process into school, for both child and family, was smooth and positive.
